Sudbury Holiday Gift Card Drive

PLEASE REVIEW THE FOLLOWING GUIDELINES BEFORE SUBMITTING YOUR REQUEST​

1. HOPEsudbury is raising money to purchase Target gift cards for families/individuals in need of assistance. All money raised will be divided equally among all applicants. We will not be able to estimate how much each family/individual will receive until the holiday drive has been completed and all donations have been collected.

2. This holiday gift card drive is open to Sudbury residents and members of the Lincoln Sudbury High School and Sudbury Public School communities only.  Additionally you may only apply to receive gift cards for family members who are living at your physical address and/or in your custody.  Please no applications for extended family.

3. Gift cards will be sent out the week of December 6th.

4. By filling out this form, you are consenting to your information being shared with HOPEsudbury. This is not a secure/confidential form.

5. Last day to apply will be December 1, 2021. 

6. Once you press "submit," a pop-up will appear below (on this screen), letting you know your request has been processed. This will let you know we have received your submission.
